12-Month NBN Uplift Agreement

This agreement ("Agreement") is made between the Customer and Uniden Voice Over Cloud (“the Provider”) for the uplift of the NBN internet service. By signing below, the Customer agrees to the following terms and conditions:

1. Service Commitment

Upon requesting and accepting the uplift of the NBN service, the Customer agrees to a minimum contract term of 12 months from the activation date of the upgraded service.

2. Plan Details

  • Service Type: NBN Internet Service (Uplifted/Upgraded Plan)

  • Start Date: [Date of Upgrade Request]

  • Minimum Contract Term: 12 months

3. Early Termination

If the Customer chooses to cancel the service prior to the completion of the 12-month term, an early termination fee will apply. This fee will be calculated as:

$[monthly fee] × number of remaining months in the contract.

4. Service Changes

Changes to the service, such as plan downgrades or cancellations, may result in termination fees and/or resetting of the contract term. Any upgrade may initiate a new contract term at the discretion of the Provider.

5. Customer Obligations

The Customer agrees to:

  • Maintain an active payment method for monthly billing.

  • Notify the Provider of any address change or service issues.

  • Use the service in accordance with fair usage and acceptable use policies.

6. Provider Rights

The Provider reserves the right to:

  • Modify service inclusions with reasonable notice.

  • Terminate service for misuse, non-payment, or violation of terms.

7. Agreement Confirmation

By signing this form, the Customer acknowledges and agrees to the 12-month minimum commitment and all terms outlined above.
